If you would like to make a tax-deductible contribution to The Play Group Theatre in honor of an occasion or in someone's memory, please send your check payable to:

On a separate piece of paper please let us know:
- for what occasion or whom the donation is in honor/memory
- the name and address of whomever you would like to receive notification of the contribution
- your name and address (and phone number - optional)
